The practice has also been designated a Patient-Centered Medical Home (PCMH). PCMH patients and their family (when appropriate) establish a partnership with their practitioners to ensure they have the support needed to make decisions and participate in their own health care.The schedule for this full-time opportunity is a traditional workweek, Monday - Friday, and may include early morning, late evening, and rotating weekend shifts to increase pat
